Please help with radiator fan issue

Car overheated a couple weeks back and realized the radiator fan was no longer working. Checked fuses and relays and everything was fine so I just replaced the entire assembly. Replacement worked perfectly until it completed stopped a couple days later.

Now just the out another replacement on and it’s STILL not working. I’m at a loss here. I’m still getting 11.8v to the harness so everything is good there. What am I missing??

Were they OEM fans that came with the module? I had a similar problem and it turned out on the Body harness side of the module the plug was not all the way in. Had to use some plyers to actually get it to snap in place. Other wise the plug would vibrate free just enough for the trigger electrode to lose contact.
